Job description

• 6 month contract to start minimum
• Ideally they are wanting/needing the provider for 2+years
• 3-4 week credentialing-- could be sooner
• 4 days a week preferred
• 8:30 first patient- last patient 4:30
• 20-30 patients per day
• nurses and all other support staff
• They will cover T&L-- if you send someone in the price range on the agreement on the account
• We are going to send them something that we sent bubbles for long term pricing- when Allie gets back
• Samantha Pulley is the Credentialing person- Sally Cesar is the main POC
• BLS,PBLS, DEA, state control
