Position Highlights:

As a Behavioral Care Manager, you will:

Actively engage with pregnant and postpartum patients in OB clinics, educating and enrolling them in FamilyWell's perinatal collaborative care program.

Process referrals, verify insurance coverage, and ensure all enrollment requirements are met.

Maintain strong patient engagement through outreach, reminders, and follow-up care, striving for an 80% engagement rate in the program.

Update patient records in EHRs, communicate care plans to clinic providers, and track patient follow-up and clinical outcomes in the collaborative care registry.

Provide patient education on perinatal mental health disorders and available treatment options.

Facilitate outside referrals for community-based social services when clinically indicated.

Ideal Candidate:

Holds a Master’s degree in Social Work (MSW or LCSW), a bachelor or master’s degree in Psychology, or a Nursing degree (BSN, RN, MSN).

Brings 2 years of prior experience supporting perinatal mental health patient populations.

Resides in Brookline, Massachusetts, or within a 30-mile radius.

Ideally completed or in the process of completing the Perinatal Mental Health Certification (PMH-C) through Postpartum Support International.

Bilingual in Spanish (and/or Brazilian Portuguese) and English is a plus.
