- July 18, 19 (clinic)
- July 20, 21 (24 hour call)
- Aug 15,16 (clinic)
- Aug 17,18 (24 hour call)
- Sept 19,20 (clinic)
- Sept 21,22 (24 hour call)
- Oct 17,18 (clinic)
- Oct 19,20 (24 hour call)
- Nov 11,12,13,14,15 (clinic)
- Nov 16,17 (24 hour call)
- Dec 19,20 (clinic)
- Dec 21,22 (24 hour call)

Work Details:
• Looking to build the PRN pool of physicians
• Support staff: 1 physician and 1 APP (support stuff for infusion therapy and MA or RN)
• Patients per day: Volume pending.
• Department description: 1 physician and 1 APP see patients in clinic and round in hospital. APP assists with patient care and answers questions regarding infusion etc.
• EMR: Epic
• Hospital bed size: 90

Practice Setting:
Hospital and Outpatient Infusion Clinic

Board Certification: Board Certified in Oncology
License required: Ohio license
Credentialing Timeframe: emergency privileges available; 90+ day full credentialing
