Job description

• Primary coverage is needed for Monday – Friday, 3P to 11P. Occasional Saturday & Sunday coverage needed, day shift.
REQUIRMENTS
• PA-C or APRN - Required
• Active IL License – Required
• BLS Certification - Required
• Availability per request - Required
• Stratum Med Cover Sheet in Word format -- Required
• Two completed reference forms per Stratum process – Required
• Clean and easy-to-credential candidates – Highly Preferred
• PROOF OF COVID VACCINE IS REQUIRED TO WORK AT THE FACILITY - Required
• BLS (AHA)
LICENSING RESTRICTION: We are only considering candidates that are ALREADY ACTIVELY licensed in Illinois.
