Will the client wait for a license: IMLC Accepted, and Active License
Rate: Hourly or Daily
Expenses: yes, Flights, hotel, rental car

REQUIREMENTS

Type of Provider(s) client will accept (Including Specialty): Hematology/Oncology Clinic Coverage
If needed, is there a collaborating physician available or in place? No
Does the Provider need to prescribe?
Is there a state specific CSR that will be needed?
Certifications required: (BC, BE, or GP) (BLS, ACLS, ATLS, PALS): BC
Background Experience:
Credentialing Requirements/Timeframe: 60+

Start Date: starting as soon as possible 9-12 months ; Until a perm is found

Work Schedule: Mon-Fri 8a-4:30 (8hr clinic day)
every week/month needs coverage Mon-Fri 8a-4:30
