Anticoagulation Clinic
Work Details:
• Support staff: Support staff: 1 NP, 1-2 pharmacists, patient coordinator(scheduling)
• Amount of coverage needed per month: 4 x 8/weekly
• Patients per day: 8-11, blend of video and in person, pharmacist does most of the telephone visits
• Patient population/age: referred patients from cardiology, transplant, renal services
o Mostly elderly population, but can range from 18+
• Department description:
o The Anticoagulation Clinic APP is responsible for the care of patients in the Anticoagulation Clinic. In collaboration with clinical pharmacists, the APP supervises anticoagulation therapy for ambulatory patients through ongoing clinical assessment, ordering and following up on laboratory testing, and modifying medications as needed to meet anticoagulation goals.
o They collaborate with other disciplines or departments to provide care coordination involving anticoagulation management, including perioperative departments, home health, and other procedural areas.
o The APP provides patient education, verbally and in writing, regarding anticoagulation medications and indications for use, medication side effects or precautions, and laboratory monitoring timing and follow up.
o APP responsibilities include managing meds, checking labs, titrating meds, a lot of in basket through Epic, and a lot of telehealth visits
• EMR: EPIC
• Special procedures: no procedures, some point of care testing if APP is experienced
• Skills required:
o MUST have long term anticoagulation medication management
o Must have 2 years APP experience
o Minimum of 6mo – 1 year of experience managing patients on anticoagulation therapies (long term anticoagulation, not hospital anticoagulation experience, must be ambulatory/outpatient clinic experience)

Requirements:
• Board Certification:
o If NP, A-GNP-C, ANP-C, FNP-C, GNP-C, AGPCNP-BC, ANP-BC, FNP-BC, GNP-BC, ACNP-BC, AGACNP-BC, ACNPC-AG, ACNPC
o If PA, must have Master’s Degree
• License required: Yes, CA
• State license required: Yes, State of CA
• DEA: yes, CA with Schedule 2
• Certifications preferred: AHA BLS
• Minimum experience required:
o MUST have long term anticoagulation medication management
o Must have 2 years APP experience
o Minimum of 6mo – 1 year of experience managing patients on anticoagulation therapies (long term anticoagulation, not hospital anticoagulation experience, must be ambulatory/outpatient clinic experience)
o Epic exp strongly preferred
Credentialing Timeframe: 60 – 90 days
