OH state License - REQUIRED, not accepting IMLC
COVID Vaccination - REQUIRED
MUST be boarded ABMS/AOA IM/FM (can be double boarded ABPS) REQUIRED
MUST have 2+ recent experience REQUIRED
NRP - REQUIRED
Please submit provider to all 3 Holzer sites (Meigs, Jackson and Gallipolis)
