Job description

• RATES- Please submit regular HOURLY rates.
• Shift type- M-F Outpatient clinic only. Once employed, rotate 1:4 or 1:5 overnight and weekend clinic call; telephone only; Calls are vetted by nurse call center.
• Reason for Coverage- High patient demand
• Setting- Outpatient only.
• Duties- Working OP clinic seeing adult primary care patients:
• Ranging from young healthy adults to medically complex end-of-life; Includes well woman exams; No OB; No peds; lots of internal med patients
• Required Procedures- None required
• Types of Visits- See above
• Schedule- M-F days.
• PPD- 18-20 in full day of outpatient clinic; depending on mix of patients; complex get longer appointment times
• EMR- Cerner
• Support Staff- MA, Receptionist, Practice Manager
• Credentialing Timeframe- 60 days upon receipt of completed credentialing packet. Outpatient only - no additional hospital application required.
• SUBMISSION REQUIREMENTS
• Temp to Perm - REQUIRED. Must be stated at time of name clear.
• Board Certification – Must be board certified in IM or FM (MD or DO). Will consider Board Eligible ONLY if they are within 5 years of having completed their training, and have made at least one attempt at their certification within 1 year of having completed their training.
• Active CA License - Required
• Clean Malpractice History - HIGHLY PREFERRED
• Fellowship- OPTIONAL
• Disclose if provider has previously worked at CHI/Dignity - REQUIRED, note at name clear.
• COVID Vaccine - REQUIRED, note at name clear.
