Job Title: Licensed Practical Nurse / Licensed Vocational Nurse
Location: Philadelphia, PA
Employment Type: Full Time / Part Time
Schedule: Monday to Friday, occasional weekends as needed
Shift Availability: Morning, Evening & Night.

Overview:

We are seeking a skilled and compassionate LVN/LPN to join our healthcare team. The LVN/LPN will provide direct patient care under the supervision of a registered nurse (RN) or physician, ensuring the highest level of patient comfort and safety.

Responsibilities:
Provide routine care such as administering medications, checking vital signs, and assisting with medical procedures.
Monitor patient health and update medical records with observations, including changes in condition.
Assist in developing and implementing patient care plans in collaboration with the healthcare team.
Provide wound care, administer injections, and handle other basic nursing tasks.
Educate patients and their families on care plans, disease prevention, and medication management.
Adhere to facility policies, procedures, and regulations to ensure patient safety and regulatory compliance.
Communicate effectively with the healthcare team, patients, and families to coordinate care.

Qualifications:
Valid LVN/LPN license in the state of practice.
Graduation from an accredited LVN/LPN program.
BLS/CPR Certification required.
Prior clinical experience preferred, especially in long-term care or similar settings.
Strong organizational skills and ability to prioritize tasks in a fast-paced environment.
