40 hours per week
Job duties: Patient Care, Medication Management, Acute care, Transitional Care, Referral Management
Must be board certified
Can be FM, IM or GER
All patients are 18+ years of age with majority 65+
14-16 patients per day
Working with 1 MA and 1 Clinician
EMR is eClinical Works
