Full Time FP Needed at Oak Street Health Center Albion, NY

Full Time
ASAP - Ongoing
Mon-Fri (8a-5p) some Saturdays possible
~15-25 patients per day
95% Appointments / Limited walk-ins
32 clinical hours and 8 admin hours
Sick and Well Visits/Basic Primary Care
Quick Credentialing: 3-4 weeks
Full Support Staff
Mileage Reimbursement or T&L (if necessary)
EMR: E-Clinical Works
Active License and DEA required
