Northwest Indiana looking for FM Providers

Job description

Immediate FM Physician, NP, or PA needed for at 6 months with chance to extend
• Will look at someone between 3-5 days a week
Great location in Northwest Indiana
Will take 3-5 days a week
M-F preferred
8-5
• Will see all ages- including potential newborns
• Will be solo provider most of time- must be comfortable with that- be ready to see whoever comes in
• Will have set schedule
• Cred can take up to 60-90 days
• prefers 12 month contract- will look at 3 months though
