Details

⦁ Candidate CVs need to have the month & date on their work experience w/all
gaps in employment accounted for.
⦁ Includes some Chronic Pain Management
⦁ Chart closure within 72 hours
⦁ Schedule = 18 PPD, minimum see 18 PPD
⦁ Monday-Friday 8-5
⦁ A mix of peds/adults (Will consider IM only)
⦁ A decision is typically made within 7-14 days
⦁ We will also accept DO
⦁ BE will be considered if truly BE (5 years or less since residency)
⦁ Active DEA needed before the start
⦁ Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely).
⦁ The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB
⦁ Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete
⦁ Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer
⦁ or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
⦁ COVID Requirement:
⦁ Primary series plus booster, regardless of the date received
OR
⦁ 1 dose of the current COVID vaccine (2023/2024 formulation)
⦁ HCO & Vendor agree to a 30-day notice for ending all assignments.
