Job description

Schedule/Shifts Times: Monday – Friday 8a – 5p (1 hr lunch) or Monday – Friday 8a – 4:30p (1/2 hr lunch), or 4 ten hour shifts with one day off during the week (day off will be determined based on Hospital needs)

*** our last provider was doing 4 -10s but that may change depending on the Hospital’s needs at the time ***

Call Requirements: None

% inpatient / % outpatient: Outpatient only

Patient Volume: 12- 15 patients per day average

Patient Ages: adults only

Types of Cases: standard outpatient care, chronic illness treatment

Support Staff: yes (several Physicians, mid-levels, RN’s and specialty Physician support)

Procedures Required: None

Type of Charting: EMR - RPMS

Requirements: BLS, ACLS (through American Heart), DEA (fee paid, schedules 2-5)

Reason for Need: staff shortage

Credentialing Timeframe: 4-6 weeks on average
