Location: Eureka, CA

Start Date: ASAP

Schedule: Monday-Friday 8-5

Work Details:
· Support staff: 1 RN, 2 MA
·Patients per shift: 18
·Patient population/age: Mix of peds/adults
·Department description: Family Medicine
·EMR: Epic

Practice Setting:
•Outpatient

Requirements:
· Board Certification: Yes
· License required: Active CA License
· DEA: Yes
· Certifications required: BLS, PAs must have CA Controlled Substance Education Course Certificate, NPs must have Furnishing License with Schedules II

Credentialing Timeframe: 45-90 Days
