Shift : Mon- Sun, evening and night shifts

The candidate must be currently registered to practice as a Licensed Practical Nurse in NYS. Previous experience working in the community and/or with persons with disabilities is preferred. The primary function is to provide direct medical care as per provider orders. The selected LPNs will be required to attend a two-to-three week training program. The LPN will then be required to report to the assigned house to care for between four and twelve individuals for a designated shift.

The LPN will make observations about the individuals' health status and report them to the assigned RN. The LPN will be required to administer medications, including injections and any tasks affiliated with safe administration such as ensuring medications are available, communicating with providers and the pharmacy, taking vital signs and obtaining finger sticks. The LPN will be required to provide ostomy care, wound care, administer enteral feedings, respiratory treatments, maintain bladder function by inserting and/or irrigating catheters and ensure all clinical items that will be needed are available.

The LPN must document all observations, interventions and communications. Record keeping will include, but not be limited to, documentation of observations, medication and treatment administration, progress notes, intake and output, vital signs, weights and various clinical logs. Additional tasks which may be assigned include assisting with personal care such as meal time assistance/feeding, bathing, dressing and grooming, lifting, transfers and positioning. Other household tasks may be assigned which assist the individual to stay in the home.

The LPN may also be asked to accompany the individual to medical appointments or social events, such as dining out or going to movies as desired and tolerated.
