Position : Social Worker
Location : Mauston, WI 53948
Duration : 8 Weeks Contract (possibility of extending after 8 weeks)
Shift : Mon- Fri 9:00 AM - 5:30 PM

Compensation Break-up as per 40 Hours/Week:
Pay Rate: $69/hr.
Housing and Meal Stipend: $1160 (TAX-FREE)
W2 Pay: $1240 (TAXABLE)
Total Pay: $2400/Week (Excluding Taxes

Job Description:
• The Social Worker position assists and supports patients and their families to identity areas of need. Works with the Case Manager of inpatient services to assist patients and their families.

Job Duties:
• Assists with discharge planning primarily in Acute Care.
• Work with IDT to identify needs.
• Discuss/arranges options, services, and resources to help with areas of need.
• Assist with Health Care Power of Attorney documents and other Advanced Directives as able.
• Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
• 1+ year of experience – Required.
• Perform other duties as requested.

Required Certification:
• CPR certification

State License Requirement:
• Wisconsin

• No Weekends
