Mon-Fri 2p-10p

Work Details:
• Facility Type: Clinic
• Practice Setting: Occupational Medicine
• EMR/Charting Software: SYSTOC
• Vaccines Mandatory?: NO
• Avg. Daily Census: 20-25
• 100% Occupational Medicine. NO Urgent Care
• DOT certified required
• Credentialing will take 3-4 weeks
• Digital x-ray onsite
• Solo provider. Will have MA and Nurse alongside
• Open to 4 days/week
• Bi-Lingual in Spanish a plus
• Common occupational musculoskeletal injuries of the neck, back, and upper and lower extremities.
• Work-related injuries to the head with or without loss of consciousness, assuming the individual is currently conscious and alert.
• Work-related injuries to the chest or abdomen, with the possibility of hernias.
• Work-related lacerations of the face, scalp, and extremities. o Work-related eye injuries, including foreign bodies to the eye, non-penetrating eye injuries, and chemical exposures.
• Work-related rashes and other occupational skin conditions.
• Occupational asthma and other work-related respiratory symptoms.
• Treatment of non-displaced uncomplicated fractures, including interpreting radiographs, splinting, and orthopedic referral when appropriate.

• Board Certifications Req’d/Preferred: Not Required
• State License(s): Yes
• Years of Experience Req’d: 2+ year

Credentialing Timeframe: 30 days
