Physician Assistant OR Nurse Practitioner Stamford, CT

We are looking for a motivated Physician Assistant or Nurse Practitioner to join our practice full time in Stamford, CT. We are looking for a Physician Assistant or Nurse Practitioner who has at least 3 years of clinical experience, with experience or interest in functional medicine and/or hormone replacement therapies preferably. Must have impeccable clinical and communication skills, with a customer-centric attitude, be organized, efficient and proactive, and a passionate, energetic team member who believes in an innovative approach to achieving optimal wellness. Duties:
• Complete thorough and holistic consultations (in-person or via tele-medicine)
• Order and review labs/diagnostics
• Develop appropriate treatment plans based on patients concerns, goals and lab results.
• Prescribe replacement therapies and supplement regimens
• Perform procedures as medically needed (training will be provided)
• Maintain charting/clinical documentation for each patient in electronic medical records (EMRs)
