Job Title: Mental Health Clinician (Nursing Home & Assisted Living Specialist)
Location: Greater Cincinnati Area (On-site at partnered facilities)
Practice: Restore at A Ray of Hope
About the Role
Restore at A Ray of Hope is a private practice dedicated to providing high-quality, "active restoration" mental health services to seniors and individuals in long-term care environments. We are seeking a dedicated Mental Health Provider to join our team, serving residents in nursing homes and assisted living facilities across the Greater Cincinnati area.
In this role, you will act as a vital bridge between the resident’s emotional well-being and their overall care plan, providing therapy and support where they live.
Key Responsibilities
Clinical Assessment: Conduct comprehensive initial diagnostic assessments and mental status examinations for residents within long-term care settings.
Individualized Therapy: Provide evidence-based psychotherapy (CBT, Mindfulness, Motivational Interviewing, etc.) tailored to common geriatric concerns including depression, anxiety, grief/loss, adjustment to transitions, and cognitive changes.
Interdisciplinary Collaboration: Work closely with facility nursing staff, social workers, and medical directors to ensure mental health goals are integrated into the resident’s overall care plan.
Crisis Intervention: Provide immediate support and de-escalation strategies for residents experiencing acute emotional or behavioral distress.
Documentation: Maintain timely and accurate clinical documentation in compliance with Ohio state regulations and Medicare/insurance requirements.
Family Support: Facilitate communication with families and guardians when appropriate to support the resident’s progress and well-being.
Qualifications
Education & Licensure: Must hold a valid Ohio license as an LISW, LPCC, IMFT, or Psychologist. (If you are hiring for med management: PMHNP or MD/DO).
Experience: Previous experience in geriatric mental health or working within nursing home/assisted living environments is highly preferred.
Clinical Skills: Strong proficiency in treating age-related mental health disorders and navigating the complexities of chronic illness and end-of-life care.
Autonomy: Ability to work independently and manage a travel schedule between various facilities in the Greater Cincinnati area.
