• 24 Hour CAll
• Board Certified Pediatrics - Required
• MI License & CSR at Submission - Required at time of clear, no exceptions - not accepting IMLC
• BLS & PALS - Required
• Must have clean background for quick credentialing- REQUIRED no exceptions
• DEA - Verbal Confirmation Required at Name Clear
• Certification Requirements
• BLS (AHA)
• Board Certified
• PALS (AHA)
• State License Requirements
• Michigan
• Additional License Requirements
• MI CRS


• PPD 17-20
• Processing time for privilege's Application: 2 weeks
• Emergency Privileges'? yes- 1-day!
• Will other Pediatricians provide coverage during locums coverage dates (list names): NO
• Midlevels providing coverage? 0
• how many pediatric intensivists provide coverage: 0
• average number of of ICU admissions: 0-1
• % of patients are nursery 10%
