MSW needed for PACE Program

Location: San Diego, CA City on the Pacific coast of California known for its beaches, parks, and warm climate. Home to the renowned San Diego Zoo, numerous art galleries, museums, and galleries. Opportunity: MSW- LSW *Full Time Employed *Work with an interdisciplinary Team for initial assessments, care planning and periodic reassessments based on participants case. Maintain current written case management records and provide in-home assessments for PACE participants and complete social service assessments for patients. Provide individual and family counseling, developing and leading group counseling. *Requirements: MSW from Accredited School of Social Work, Valid CA Driver’s License and access to an insured vehicle for home visits *Preferred: Clinical Social Worker Experience, 1 year in health care setting and experience working with elderly patient population *Bilingual English/Spanish Preferred Compensation: Competitive Hourly Rate with full comprehensive benefits.
