Consultant Psychiatrist --Integrated Behavioral Health

Overview:
We are seeking a board-certified psychiatrist to join Mederva Health.
As Mederva’s Psychiatric Consultant, you will be the clinical expert behind our behavioral health program, ensuring that the patients who need the most guidance get it — routed efficiently through their own primary care physician, covered by insurance, and without the barriers of traditional psychiatric referral.

This role sits at the intersection of scalable care design and high-acuity clinical judgment, and is central to Mederva’s mission of making proactive, comprehensive care accessible to everyone. It is ideal for a psychiatrist passionate about collaborative care, preventive behavioral health, and expanding access to psychiatric expertise at scale.

Requirements:
- MUST be Board-Certified in psychiatry.
- MUST have multi-state licenses to practice- minimum 5. Prefer 10 state licenses or more.
- High preference given to applicants with license to practice in Hawaii, South Carolina or Arkansas.

## Key Responsibilities

### Patient Registry Review & Oversight

- Maintain ongoing oversight of the enrolled CoCM patient panel, identifying patients who require psychiatric attention, including those:
- Not improving as expected based on validated outcome measures (PHQ-9, GAD-7)
- Presenting with diagnostic uncertainty or complex psychiatric comorbidities, including PTSD, addiction, and co-occurring conditions
- Carrying elevated suicide risk or active safety concerns

### Clinical Consultation & Treatment Recommendations

- Conduct asynchronous weekly caseload review of charts flagged by the behavioral health team for psychiatric consultation, with recommendations documented directly in the chart
- Hold weekly office hours to discuss urgent or high-acuity cases with the behavioral health coaches or clinical team — including patients presenting with suicidal ideation, significant mood dysregulation, or other clinically concerning presentations requiring immediate guidance
- Advise the care team on psychiatric diagnosis, including differential considerations for complex presentations
- Recommend psychotherapy modalities (e.g., CBT, DBT, trauma-focused approaches) when clinically indicated
- Provide guidance on medication selection and titration for PCP-initiated pharmacotherapy, with clear rationale and monitoring parameters
- Provide safety planning frameworks and risk management guidance to the behavioral health coach and clinical team for patients with elevated psychiatric risk

### Care Escalation & Referral

- Determine when a patient’s needs exceed Mederva’s current care model and require referral for outpatient psychotherapy, psychiatric evaluation, or higher-level care
- Route formal referral recommendations to the PCP through Mederva’s established care coordination workflow

### Program & Quality Contributions

- May be periodically asked to provide clinical feedback to the care team, including input on protocol refinement or escalation criteria
- Participate in scheduled case conferences or team trainings to strengthen the coaching team’s clinical competency
- Collaborate with Mederva’s Medical Director on policies governing psychiatric scope within the care model


## Role Limitations

- **No prescribing or medication management** — Recommendations are advisory; all prescribing decisions rest with the patient’s PCP
- **No direct patient care*
