Assignment Length: 26 weeks
Shift: Day Shift – 5x8s (Monday–Friday, 8:00 AM–4:30 PM)
Schedule Details
Call: Office call only (after hours and weekends)
Float: Coverage required at the ambulatory clinic and Clifton Springs OR space
Patient Acuity: Ambulatory only
Support Staff: Secretary and LPN onsite during clinic days
Lunch Break: Typically 12:00 PM–1:00 PM
Required Procedures
Candidates must be proficient in:
Epidural Steroid Injections (cervical, thoracic, lumbar)
Medial Branch Blocks (cervical, thoracic, lumbar)
Radiofrequency Ablation (cervical, thoracic, lumbar)
Nerve Blocks (various anatomical sites)
Trigger Point Injections
Sacroiliac Joint Injections
Spinal Cord Stimulator (trial placements only)
Genicular Nerve Blocks and RFA
Intercostal Nerve Blocks
Piriformis Injections
Requirements
Board Certification: Required
Minimum Experience: 1–2 years post-fellowship
NY State License: Active and in hand
Certifications: BLS
EMR: Epic experience required
Must have updated work history with no gaps greater than 30 days
If there is any malpractice history (dismissed or pending), a physician statement is required

Position Overview
The provider will deliver high-quality healthcare services and participate in quality assurance initiatives. Responsibilities include direct patient care, call coverage, coordination of diagnostic and treatment plans, and collaboration with clinical and administrative leadership to ensure efficient, accessible, and quality patient care.
Additional responsibilities include:
Supervising and instructing nursing and support staff
Participating in peer review and quality assurance activities
Serving as faculty for residency and medical student programs (as applicable)
Complying with hospital, local, state, and federal regulations
Maintaining professionalism and a collaborative work environment
Age-Specific Competency Requirements
Provider must demonstrate competency in treating:
Neonates (<1 month)
Infants (1 month–1 year)
Children (1–12 years)
Adolescents (13–17 years)
Adults (18–64 years)
Geriatrics (65+ years)
