Job description

Start Date: ASAP
Contract Length: 6 months

Schedule:

Tuesday, Wednesday, Thursday, and Friday: 8:30 AM - 5:00 PM
1st and 3rd Monday of the month: 8:30 AM - 5:00 PM
2nd and 4th Monday of the month: 8:30 AM - 8:00 PM
2nd and 4th Saturday of the month: 8:30 AM - 1:00 PM
Details:

Patients per Day: 20-24 patients
Patient Types: General pediatrics, well and sick baby visits, physicals (ages newborn to 17)
Procedures: Circumcisions
Support Staff: Provided
EMR: Athena
Practice Setting: Federally Qualified Health Center (FQHC)
Board Certification: BE or BC
State License Required: IL license required
Experience: No new grads
Credentialing Timeframe: 30-60 days
