• Shift/Schedule: Clinic and evening call during the week, 24hr call on weekend (4hrs gratis on weekend, 8hr gratis during the week)

Requirements:
• Board Certifications: BC
• State License(s): Prefer Idaho or IMLC
• Certifications: n/a
• CSR Req’d: Yes
• DEA Req’d?: Yes
• Years of Experience Req’d: 2 + years
Credentialing Timeframe: up to 90 days
Willing To License: Yes, but preference is IMLC

• 13 weeks with option to extend
• On call required
