Job description

- Dates coverage is needed: ASAP – ongoing (min 6 month commitment rqr’d)

- Schedule/Shifts Times: Monday – Friday 8a – 5p (1 hr lunch) or Monday – Friday 8a – 4:30p (1/2 hr lunch), or 4 ten hour shifts with one day off during the week (day off will be determined based on Hospital needs)

- Call Requirements: None

- % inpatient / % outpatient: Outpatient only

- Patient Volume: 12- 15 patients per day average

- Patient Ages: adults only

- Types of Cases: standard outpatient care, chronic illness treatment

- Support Staff: yes (several Physicians, mid-levels, RN’s and specialty Physician support)

- Procedures Required: None

Requirements: BLS, ACLS (through American Heart), DEA (fee paid, schedules 2-5)

Credentialing Timeframe: 4-6 weeks on average
