Requirements:
Board Certifications: Required
State License(s): Must have a valid Oregon (OR) license
Certifications: Advanced Cardiovascular Life Support (ACLS)
CSR Required: No
DEA Required: Yes
Years of Experience: Minimum of 2 years
Credentialing Timeframe: 60-90 days
