Nurse Practitioner Natick, MA (25 miles W of Boston)
We are looking for a wound care experienced Nurse Practitioner to join our medical group in the Eastern MA area. This position can be part time, with the potential for full time at 20-40 hrs per week depending on case load, locations covered, providing essential wound care services at various assisted living facilities and skilled nursing facilities across Eastern Massachusetts, with the potential to work both the North Shore and the South Shore for more hours. Must have a Wound care certification or substantial wound care experience. We cover facilities in the following communities, Natick, Amesbury, Danvers, Saugus, Bridgewater, Abington, and Fall River. Rounds and schedule will vary depending on patient need and census volume, with some locations not needing rounding upon every week.

Responsibilities:
• Deliver comprehensive, patient-centered wound care to residents of assisted living facilities, patients of skilled nursing facilities and home care.
• Perform comprehensive wound assessments and develop treatment plans.
• Implement wound care therapies, monitor patient's response to treatment, and adjust treatments as necessary.
• Communicate effectively with patients, families, and facility staff regarding treatment plans and progress.
• Document all treatments and care provided accurately and thoroughly in electronic medical records.
• Comply with all federal and state regulations regarding wound care treatment and patient privacy.
