$65 - $70/hour
We are looking for a caring Nurse Practitioner to join our medical practice in the NW Chicago area providing wound care. We need a compassionate practitioner, who enjoys helping others, and is interested in the field of wound care. We are looking for Nurse Practitioner to treat patients, focusing on wound care in home health setting. This position will start as part time, 1-2 days per week with potential for 3rd day. The Nurse Practitioner will be covering the Huntley, Schaumburg, Elgin, Carpentersville, Cary, and Barrington areas.
Job Description:
• Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources
• Evaluates and manages patients for wound care and ensuring delivered care meets current standards of practice
• Adheres to clinical practice guidelines for wound care
• Performs care that is accurate, efficient and consistent with the patient’s needs
• Coordinates the patient plan of care with the facility’s clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals
• Ensures that education is provided to the agency staff regarding the management of the patient’s wound(s)
• Maintains communication and a working relationship with home health agency Administrator, team of health care providers and patient family members regarding wound care services
• Demonstrates competency in the position of wound care including assessment, debridement, wound dressings, skin substitutes and other treatment as needed
• Explains treatment plan to patient and family members based on company standards
Required Skills:
• Must be certified in Sharps Debridement or have debridement experience
• Must be wound care certified or have relevant experience
• Previous wound care experience required
