REQUIREMENTS:
Board Eligible (must obtain certification within 3 years of employment) or Board Certified in Family Medicine - REQUIRED
ID license (or willing to obtain) - REQUIRED
CSR/CDS (or willing to obtain) - REQUIRED
DEA (or willing to obtain) - REQUIRED
Clean license and malpractice history - REQUIRED
On Call Requirements: Practice call shared with other eligible FM providers and is approximately 1:40
Weekend Requirements: OPTIONAL. Practice call shared with other eligible FM providers and is approximately 1:40
