Job description

• Family Medicine Physician Needed
• Must Have At Least 5 Years of Geriatrics Experience
• Board Certified by the American Board of Family Medicine
• Monday through Friday
• 8A to 5P
• No more than 18 patients
• 2 Advanced Practitioner for Support
• EPIC EMR
• 90 Credential
• Provider cannot start until fully credentialed
• Must have active DEA and Active Oklahoma License
• Travel & Lodging built into an all-in rate

• ASAP
• On-going
