BC MD/DO or NP
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
NP either Family Medicine or Adult/Geriatric specialty trained.
ASAP start
Monday - Friday
7:30 - 5:00
All adults, 65 and older
18 - 23 patients per day
Duties: medication management, chronic care management health assessments, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs XRays, Labs and Pharmacy in house (no narcotics filled here, patients fill controlled substances with outside pharmacies)

• 2/6-5/5 Mon-Fri 7:30-5
• patients 65 and older 18-23 patients a day
• 100hr negotiable
