ASSIGNMENT DETAILS: JO# 2635522
• Location: Lebanon, IN
• Start: December or January
• Length of commitment: 3-6 months with the option to extend
• Shifts: flexible on shifts; 4x10s or 5x8s
• Schedule Details: Outpatient, Full time, 40 hours/ week, 36 clinic and 4 admin, weekends only if on call.
• Call: 0-5 calls
• # of patients per day: 16 – 20 patients/ day
• Patient Age Group: 18+
• Support staff: Nursing and MA’s in office.
• EMR: Meditech Expanse
• Requirements: Board Certified, Active IN license, DEA, BLS, ACLS, 1 year experience
• Travel, lodging and malpractice covered
