Location: Chicago, IL
Start Date: Sept (preferred)
Schedule: Mon-Sat (one Saturday shift per month)
Department Description: Family Practice - all outpatient.
Patients age: 0-99 (Peds may not be required)
Credentialing Timeframe: 45+ days
