Practice Setting:
• Outpatient

Requirements:
· Board Certification: Certified
· License required: Active CA License
· Minimum experience required: 3 years
· DEA: Yes
· Certifications required: BLS

• $115 - $130/hr
• Open 5 days a week, Mon-Fri 8am-5pm
• Ongoing need, guaranteed hours
