Job Description

The primary focus of the Nurse Practitioner will be to provide in home medical care to high-complexity patients.

Details:

Full-time, permanent position
Perform an initial comprehensive assessment on all new members
Manage patient care on a longitudinal basis
Diagnose, prescribe and treat within the scope of care
Partner with the primary care provider to deliver integrated care
Participate in rotated on-call

Completion of an accredited Nurse Practitioner program
Minimum 2 years clinical experience
