Facility Coverage- 24 MD/ 10 APP

Locum Request for: EM PA

MD/DO Certification Requirements- Active State medical license, CS, DEA with IL address, board certified, EM, IM, FM does not accept ABPS. ACLS, ATLS,PALS. CME pas 2 years, copies of all education training diplomas, driver's license. Complete hospital provider enrollment forms, original mailed to MSO *references to be completed on hospital specific forms? forms available via DocuSign*

Merit Badge- ACLS, ATLS, PALS

EMR- CPSI

Shift Preference- ALL

Shift Times- APP shifts 8a-6p MON-FRI all of march is open

All in Hourly BILL Rate- Send your best

Interview required

• 6k Annual Volume
• Will cover Travel and Lodging
• Mon-Fri 8-6 (will consider part time)
