**Salt Lake City, UT | Heart Failure NP/PA | November Start | 13-Week Assignment**

* Start: November
* Contract Length: 13 weeks
* Schedule: Monday–Friday, 7:00 AM–4:00 PM
* No call
* Inpatient rounding only
* Coverage for maternity leave (November–January)

Practice Details:

* Heart Failure service
* Inpatient heart failure management
* EMR: Epic

Requirements:

* Nurse Practitioner or Physician Assistant
* Heart Failure Certification required
* Active Utah license
* Active Utah DEA
* Inpatient heart failure management experience preferred
