Assignment Details:

Start Date: April 21

Contract Length: Ongoing

Schedule:
• Monday–Friday, 7:00am–5:30pm in the Cath Lab
• Every Wednesday: 8:00am–5:00pm In-Office Clinic
• 2nd Fridays: 8:00am–5:00pm Telemedicine Clinic
• 4th Fridays: 8:00am–5:00pm In-Office Clinic (starting at 9:00am)
• Friday night beeper call

Available Dates:
Apr 21–25, Jun 9–13, Aug 25–29, Sept 22–26, Sept 29–Oct 3, Oct 13–17, Oct 20–24, Nov 17–21, Nov 24–26 & 28, Dec 15–19, Dec 22–26

Practice & Patient Info:

Support staff: TBD

Clinic: 20–22 patients/day

Cath Lab: 3–4 consults + 4–6 EP cases/day

Adult population

Hospital Bed Size: 268

EMR: Cerner

Procedures: Cardiac Electrophysiology with a focus on Ablations

Must show 100 EP cases (with ablation emphasis) over the last 2 years

Requirements:

BC or BE in EP

AZ license or willing to obtain

DEA (AZ registered)

ACLS certified

Minimum 1 year of experience

• Ongoing for 6 months
• Flexible Dates
• Willing to license
