Types of Cases required: General, Ortho(mostly spine), urology, ophthalmology, GYN, very few dental and peds dental, ENT, podiatry, and heavy OB

Must be proficient in Spinals, blocks, Epidurals: Yes

Case logs required? Yes - proof of epidurals, spinals and blocks

6 ORs, 2 ENDO, 0 CV, 1 CATH LAB, 1 IR Room

**This is not a supervision model. CRNAs work independently.

EMR- EPIC

License required: Yes, will accept IMLC

Additional Certs required: ACLS, BLS, PALS

Name Clear- Yes (Full Name, Job/Specialty, NPI#, Site)

Credentialing Time Frame- TBD, License needed in order to start credentialing process,

Will not take Board Eligible, Must be Board Certified
