April 12 -13, May 12-16, May 12 - 18, May 27 - 30, June 2-6, June 9-13, June 23- 27, June 30 - July 3, July 7-11
July 14-18, July 14-20, July 21-25, July 28-August 3, August 4-8, August 4-15, August 18 - 31, September 15-19,
September 29 - October 3, October 6 - November 2, October 27-31, November 10-14, November 17-21, November 17-28, December 1-7, December 8-12, December 8-19, December 22-24, December 26-31, December 29-January 4, 2026

7a - 3p / Call hours: 3p - 7a

2 weeks on 2 weeks off (MD's surgical call every day while working)

Call Response Time: 20 Minutes

***Client looking for 1 week per month of availability at a minimum. Dates from June 9th-end of 2025 might shift, looking for flexibility.

*** Call could be full 7 days, 5 days or every other night (dependent upon what provider is willing to offer)

Type of cases required: ENDO, OB/GYN, PEDs, URO, ENT, Ophthamology, ORTHO

NUMBER OF ORs- 5

Supervision: Supervision & perform own cases

Model 1 MD to 3 CRNAs

Spinals, Blocks, Epidurals - must be proficient: Yes (case logs required)

Caselogs required: Yes - proof of epidurals, spinals and blocks

EMR System: Medaxion

Vaccination Required: Need to be vaccinated / accepting religious / medical exemptions

*Some flexibility on rate TBD*

Will not take Board Eligible, Must be Board Certified
