**$30,000 ONE TIME COMPLETION BONUS**

Job description

Call bonus as follows:
1.) Weekends: When the provider covers one weekend of calls (both Saturday and Sunday), they will receive a completion bonus of $3K ($1,500 per call).
2.) Weekdays: If the provider covers 4 or more weekday calls within a month, they will receive a completion bonus of $750/call.

- Shift Type: 8 hour.
- Regular Hours: Required.
- Weeknight - On Call (Pager): Required.
- Weekend - On Call (Pager): Required.
- Overtime Expected: Yes.
- Call is a requirement, no exceptions! 6-10 call shifts per month. Call back rate is less than 10%.
- Cases: General, Ortho, OB, Colorectal, Cataracts, cystoscopy, ENT, GYN, podiatry, thoracic, GI, pacemakers, TEE, cardioversion.
- Site Charting System: iPro, Paper and pen.
- Number of Anesthesia Rooms: 5.
- Are MDs supervising CRNAs: Yes.
- What is the CRNA to MD ratio: 3/4.
- Are MDs doing their own cases: Yes.
- Credentialing: 30 days.
