Opportunity:
Physician - Internal Medicine / Family Medicine / Geriatrics

Job Details:

• Provide high quality, patient-centric, cost-effective inpatient level of care in the home setting
• Work within a team environment alongside Physicians, NPs, and RNs in a virtual Command Center
• Deliver integrated care in coordination with primary care providers, specialists, and ancillary departments
• Utilize state-of-the-art equipment available at the Command Center and in patients' homes
• Support patients with access to home infusion, meals and nutrition consults, pharmacy and medical management, mobile imaging, laboratory services, and DME and oxygen
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics required
• Committed to providing high quality, evidence-based medicine to a diverse patient population
• Excellent clinical and communication skills required
• Demonstrated ability to work effectively in a team environment required
