Hi! I have a great position for Home Health in Lane, Multnomah & Washington counties in Oregon

Contract Length: 3 months with option to extend
Schedule: Full‑time or Part‑time

Position Overview
We are seeking experienced NPs to perform home‑based and telehealth health assessments throughout Oregon. This is an assessment‑only role—no prescribing, no treatment, and no ongoing patient management.
Providers may work full‑time (40 hrs/week) or part‑time (20 hrs/week) depending on availability.

Schedule & Workload

Full‑time: Monday–Friday, 8:00am–5:00pm (1‑hour lunch)
Part‑time: 20 hours per week

Schedules released 2–3 weeks in advance
Orientation: 1–3 hours paid

Additional training modules completed during downtime within scheduled hours
Role Responsibilities

Conduct 5–6 in‑home health assessments per day
Perform annual risk assessments and chronic condition reviews
Assessment‑only (no treatment, prescribing, or ongoing management)
Perform A1C testing when applicable
Escalate urgent issues to the patient’s primary care physician
Complete documentation accurately within required timeframes


Requirements

Active NP license
Must be enrolled/credentialed in Medicaid
Experience with home health, telehealth, or chronic condition management preferred
Reliable transportation for in‑home assessments when needed
Comfortable working independently in both virtual and in‑home settings
