Palliative Care APP needed in Medford, OR

Job description

We have a palliative care APP opportunity available starting in March in Medford, OR. Please see the details below:

✅ Setting: Hospice
✅ Shifts: 5x8 + call - (average 4 nights of weeknight call and 1 weekend a month)
✅ Work Details:
· Support staff: Yes
· Providers Requested: 1
· EMR: Epic
· Skills required: Admissions and recertification face to face visits. They will be primary for symptom management for patients including medication prescribing. Partner with RN's for collaboration on patient needs
✅ Credentialing timeframe: 30-60 days

Requirements:
📌 OR state license in hand
