Job description

Hours: Tuesday-Thursday, 8-5,
Family medicine
See all ages
15-17 PPD
Must have an active Medicaid in IL
Certifications needed: BLS ,AHA
Typical Cases: Physicals, prompt care
Travel and lodging: on a per-case basis, would be mileage only
Support staff: LPN, PT Doc,
Cred timeframe: 2 weeks
Needs transcripts, vaccine records, and peer references
Closed on: Columbus day, veterans day, thanksgiving, Christmas, and New years Day

• 3 days a week at a community health center- Tues, Wed, Thurs
• Mileage covered for a local candidate
• Basic primary care- seeing all ages!
